- Go to that manufacturer's PAP page (listed above) and download the application.
- Most forms need: proof of income (tax return or pay stubs), insurance status, and a prescriber signature.
- Ask your doctor's office — many have a financial navigator who files these for you.
- If denied or you have Medicare, apply to a foundation (PAN, HealthWell, PAF) for your disease.
