Healthcare.gov — LGBTQ Coverage

Marketplace plans cannot discriminate based on sexual orientation or gender identity. Coverage of gender-affirming care varies by plan — compare carefully.

Before your first appointment with a new provider

  1. Call ahead: ask if they use chosen name and pronouns in the chart, and whether intake forms include nonbinary options.
  2. Confirm they're in-network — and that any specialist they refer to (endocrinology, surgery, mental health) is also covered.
  3. For gender-affirming care, ask what their letter or readiness process looks like and whether they follow WPATH Standards of Care.
  4. Bring a copy of your insurance plan's "gender dysphoria" or "transgender services" coverage section — exclusions vary widely.
  5. If you've been denied coverage before, save the denial letter — Trans Health Project and Lambda Legal can help appeal.
  6. For HIV/PrEP: ask about lab costs upfront and whether the clinic enrolls patients in Ready, Set, PrEP if uninsured.
  7. If you experience discrimination, document it and file a complaint with HHS Office for Civil Rights under Section 1557.
