Starting the Call

Ask for the representative's name and write it down.
Ask for a call reference number right away.
Explain your question or problem clearly.
Give them your policy number and date of birth when asked.

Questions to Ask

Can you explain why this claim was denied?
Is there a problem with the billing code?
Is this provider in my plan's network?
What steps do I need to take to fix this?
Do I need to file an appeal?
How long will it take to get an answer?

Before You Hang Up

Read back what the representative told you to make sure you understand.
Ask what the next step is and who needs to do it.
Ask when you should call back for an update.
Make sure you have the reference number written down.
Keep your notes in a safe folder.
