Copies of the front and back of your insurance cards.
A copy of your driver's license or photo ID.
A printed copy of your insurance plan's summary of benefits.
The Medicare or Medicaid welcome letter if you have it.
Contact numbers for your insurance company.

Medical Records

A copy of your current medication list.
Vaccination records and dates.
Results from recent blood tests or x-rays.
Discharge papers from any hospital stays.
Notes from your last doctor visits.

Billing and Financial Papers

Unpaid medical bills organized by date.
Explanations of Benefits (EOBs) from your insurance.
Receipts for medical bills you already paid.
Letters about charity care or financial aid.
Records of any medical-debt credit reporting changes.

Legal and Planning Documents

Your living will or advance directive.
Medical power of attorney forms.
A signed HIPAA release form for your family members.
Your emergency contact sheet.
Any paperwork for ERISA external review rights if you are doing a big appeal.
